be great if placing your subacute patients could be done with just
one telephone call? At VitalCare America
(SM), we pride ourselves on our quick response time and our ability
to assist you in placing patients with minimum delay. By dialing
1.800.SUB.ACUT(E) (1-800-782-2288), you can quickly connect to a
Provider Relations Specialist who will determine your area Case
Manager. Once determined, your request will be immediately dispatched
to a representative in your area who will call you to arrange a
time to conduct a physical evaluation of your patient.
On the day of the evaluation, our representative Case Manager will
tell you whether or not we can work with you in placing your patient.
If for any reason we are unable to meet your placement needs, we
will be happy to assist you in finding a more appropriate placement
option. If your patient is appropriate for placement in a VitalCare
America (SM) Unit, our Case Managers and Provider
Relations team will be happy to coordinate the benefits and funding
arrangements for you.
