VitalCare America is dedicated to developing alternative methods of care for patients with complex medical needs. This dedication stems from the belief that medically complex patients have unique care needs that cannot always be met in a traditional acute care unit. These needs include intensive therapy, emotional support and an individualized care plan developed by a comprehensive and caring medical team. Staffing for subacute care is slightly lower than acute care, yet is significantly higher than the staffing for skilled nursing facilities. This enables our care givers to devote the time necessary to care for those patients who have more intensive medical and social needs. Our Subacute Care Program provides a Medical Team consisting of a Physician, Clinical Nurse, Social Worker, Dietician, Respiratory Therapist, Physical Therapist, Activity Coordinator, and other specialized personnel. Our caring professional staff holds a monthly VitalTeam (SM) Conference to review each patient's plan of care and medical treatment. Family members are encouraged to attend the monthly VitalFamily (SM) Network Meetings to learn first-hand how their loved one is progressing. Our goal is to maximize improvement for each patient and to provide support for the entire family.


Family Support Group

VitalCare America has established a policy of having Family Support Groups at each of our subacute units. Our staff is here to help the families and friends of our residents with their problems and concerns, just as we are here for the residents. As a friend or family member of a new resident to the subacute unit, Family Support Group Meetings can offer an opportunity to learn about how the unit runs - who does what - and how to get your questions answered. More importantly, you will learn how you fit into the care of your loved one. We will help you to understand the importance of your involvement in improving the quality of life for your loved one. At the same time, we will try to help you understand your own limitations and your need for a life separate from that of your loved one who is living on the unit. The Family Support Group also gives you an opportunity to help you understand the complexities of issues relating to your loved one on the unit. Speakers from the community are often invited in to discuss issues such as Medi-Cal, conservatorships, medical aspects of subacute care and other issues of interest to the families. More than anything else, the Family Support Group gives each friend and family member the feeling that he or she is not alone in facing and dealing with the complex and sometimes painful issues of having a loved one in need of long-term care.
